Templates for tattoo, piercing and PMU

Tattooing, piercing and permanent makeup carry different risks, different aftercare and often different local rules. If you do more than one, use more than one template.

Where the disciplines differ

Build one template per discipline

The cleanest setup is one template per thing you offer:

1. 1Build your most-used template first and get the consent text right.
2. 2Duplicate it, rename it for the second discipline, and change the questions and consent text that differ.
3. 3Repeat for the third.

Duplicating is much faster than starting over, because your studio name, your general terms and your standard health questions carry across. See Edit, duplicate or delete a template.

Give each one its own link and QR code

Each template gets its own share link and QR code, so a client scanning the code at your piercing station gets the piercing form. Print them separately and label them clearly. See Print and place a QR code.

What to change per discipline

Piercing: ask about metal allergies and previous rejection, and drop the tattoo placement and description fields in favour of your own questions about the site and jewellery.

PMU: add questions about previous permanent makeup, existing pigment, and any recent facial treatments. Set expectations about colour settling and the retouch appointment in your consent text.

Tattoo: keep placement and description, and add anything specific to the style you work in.
