Personalised forms are rolling out gradually, starting with new accounts. Without them you can start from Wavrr’s standard tattoo, piercing or PMU template.
Is the form legally checked?
No. It isn’t legal advice. Review the wording and make sure it meets your licence and your insurer’s requirements.
Can I make more than one?
Yes, one per service. You can start up to 10 personalised forms in 24 hours, and create as many templates by hand as you like.
