Add health and medical questions

Collect the health, allergy and medication information you need before a session, without over-collecting.

Health screening is usually the main reason a consent form exists. Wavrr gives you two ways to collect it.

The built-in medical conditions field

In your template's optional fields, switch on Medical conditions. This adds a free-text box under the Health & tattoo information section of the form where clients can describe anything relevant.

Free text is flexible but hard to scan later. It works well as a catch-all alongside specific questions, and poorly as your only screening.

Specific questions get better answers

A client who is rushing will leave a free-text box empty. The same client will answer a direct yes/no. Add custom questions for the things you genuinely need to know:

  • Are you taking any blood thinners?
  • Do you have diabetes?
  • Are you pregnant or breastfeeding?
  • Do you have any allergies to latex, dyes or metals?
  • Have you had a reaction to a tattoo or piercing before?
  • Have you consumed alcohol in the last 24 hours?

Add these as yes/no custom questions, and keep a free-text Medical conditions field at the end for anything you did not think to ask.

Only ask what you need

Health information is sensitive personal data. Every question you add is data you now store, secure and are accountable for.

What happens to the answers

Answers are stored on the waiver record and appear on the signed PDF alongside the client's signature, so the disclosure and the consent are one document. You can find them again under Customers — see Find a client's consent history.
